Strongly recommend repairing or re-coring your OEM radiator rather than any aftermarket replacement.

Yes, you need a thermostat in the upper radiator hose. Secure in place with hose clamp.

Replace hoses, mounting pads, and hood patch.

4 psig cap is sufficient.
